Here you will find some of the necessary forms required throughout the course of your claim with Canstar Restorations. Please ensure you have the Work Authorization Form filled out prior to any work commencing on your property.

Customer Information Package
Explanation of the Restoration claim process from start to finish.
This form must be signed before each portion of the claim/job by the policyholder/property owner (not the tenant) before we can begin your restoration process.
Certificate of Completion
This form must be signed by the policyholder/property owner at the end of the restoration work to validate the workmanship quality.
Disposal Authorization Form
This form must be signed so that we may dispose of the items listed on the ‘Schedule of Loss’ form.
